At Self‑Propelled Health, Lifestyle Prevention Medicine is at the heart of how we help adults build sustainable, long‑term wellbeing. Offered by an American College of Lifestyle Medicine (ACLM) board‑certified nurse practitioner, we provide evidence‑based guidance that supports disease prevention, symptom reduction, and whole‑person vitality. We aim to support your efforts in preventing or reversing conditions such as Type 2 diabetes, high cholesterol, high blood pressure, fatty liver disease, obesity. With prevention and reversal comes improved mental health, physical energy and overall improved quality of life. 

We offer provincially-funded laboratory and/or diagnostic testing, blood glucose monitoring and lowering strategies, results review with our practitioner and treatment plans, to move you forward toward better health.

  • Acute Care Availability: Acute care services are available only to existing patients who have established care with us.

  • New Patient Requirement: We do not offer standalone acute care. New patients must first be seen for one of our core focus areas: Women’s Health, Chronic Disease /Obesity Management, or ADHD Assessment & Management.

  • Primary Care Context: Our specialized support is intended to complement, not replace, your ongoing relationship with a primary care provider.
